**Kaiser requires a referral. 

Superbills are sent on request or monthly when sessions occur. A superbill is a fancy word for a receipt, if you have an insurance I am not in network with you may submit this receipt to them and they will *usually* reimburse you for the cost of therapy. Please reach out to your individual insurance provider to verify they offer out of network benefits and what their requirements are for submitting superbills. I do not sent the superbill to your insurance, I will be sent to you directly. 

If you’re using insurance through Grow or Headway your copayments will be collected via Credit/Debit through their site. 

If  are a private pay client I use IvyPay, for more information about IvyPay click here, they accept all major credit and debit cards along with FSA and HSA cards. I do not accept check or cash at this time. If you need an alternative payment method please let me know and we can discuss your individual needs. 

If you are using in-network benefits the amount you pay is dependant on your plan. I will never collect more than your copay or co-insurance. Benefits are verified using your insurance’s online portal. If the information provided to you seems incorrect, let me know right away. If you have insurance and choose not to use your benefits please let me know as additional consent forms are sometimes required depending on plan. 

You are under no obligation to participate in on-going therapy. If you are just interested in an assessment and diagnosis let me know. Depending on your unique situation we may or may not be able to use your insurance benefits. If you are already seeing another therapist some plans will require you stop services with them in order to see another therapist. 

Yes, I am trained in how to evaluate for the need of an emotional support animal, therapy animal, and service animal. I can provide letters to request housing accomodations, work accomodations, or recomendations for acceptance into service dog training programs.

However, I will only write these letters for current clients I have had sessions with in the last 90 days, who have been my client 30 days or longer, who have attended a least 4 sessions, and who I have seen engage with their ESA or another animal during our sessions. If the letter is for a new animal to come into the home I may also require others who live with you to attend one session. 

Great question! YES! I do accept Kaiser Nor Cal, however there are some extra hoops. Here’s how you get a Kaiser referral. 

  1. Call your local Kaiser mental health department. Ask to speak with a triage nurse. 
  2. You’ll most likely get a voicemail box, leave your name, phone number, kaiser number, and a brief message saying you’re calling to get mental health services. 
  3. When the nurse line calls you back they will screen you for safety, ask if you’re having any thoughts of harming yourself or others and why you need therapy. 
  4. They will then schedule you an appointment with a therapist for a mental health evaluation. If you’ve already been seen at Kaiser mental health department you can skip to step 6 and ask the nurse for a referral. 
  5. You’ll receive an appointment with a therapist at Kaiser, at the end of the appointment they will say something like “you qualify for therapy, I can get you scheduled now with a Kaiser therapist.”
  6. Let them know you would like a referral to see an outside therapist through Headway (or Grow Therapy for Kaiser Medi-Cal), with Marty Hosier, LMFT 138146 (AKA Martha Hosier) NPI 1730726407
  7. Then you will need to create a profile on Headway/Grow therapy, enter your Kaiser # 
  8. E-mail me to let me know you’re ready to book your first appointment, I will open my Headway/Grow calendar, and you will book your first appointment on Grow/Headway
  9. For follow up appointments please use my google calendar, and the Grow calendar sometimes double books me, which is never fun. Thank you!! 

Yes! I do have a sliding scale for uninsured individuals who are low income or experiencing intense hardship, for more information and to apply click here. At this time I am unable to waive any copays or co-insurance if you are using your insurance benefits for therapy.

Applications are subject to verification of income, inability to obtain insurance, and if therapist is accepting new clients. Approved applicants may be subject to wait list if therapist schedule is full at time of application.
